Item Computed tomography based evaluation of prostatic fiducial marker migration between the periods of insertion and simulation(AVES, 2017-01-01) Arpaci, Taner; Ugurluer, Gamze; Ispir, Emine Burcin; Eken, Alper; Akbas, Tugana; Serin, MeltemObjective: The aim of this study was to determine whether significant fiducial marker migration occurs between the periods of prostatic marker insertion and computed tomography (CT) performed for radiotherapy planning and if a waiting period is necessary. Material and methods: Thirty-nine patients with prostate adenocarcinoma underwent fiducial marker insertion before radiotherapy between June 2013 and December 2015. Three markers were inserted by one radiologist under the guidance of transrectal ultrasonography. All patients underwent CT three hours after insertion to confirm the number and position of fiducial markers. Radiotherapy planning CT was performed on an average of 11 days (range 7-20) after insertion. CT images were imported into treatment planning system to analyze the position of fiducial markers. Point-based marker match algorithm was used to find the distance of marker migration. The mean and maximum distances between each fiducial markers were calculated. Results: The mean distance of migration was 1.029+/-0.42 mm (range 0.23-1.93 mm) and the maximum distance was 1.361+/-0.59 mm (range 0.25-2.74 mm). The distance of marker migration was not statistically significant for the groups organized according to the timing of marker insertion, prostate volume, patient age, prostate specific antigen level and Gleason score. Conclusion: According to our results significant fiducial marker migration did not occur during the interval between insertion and treatment planning CT. It should be taken into consideration that performing simulation on the same day as marker insertion might prevent increased cost and delayed radiation therapy by saving the patients from extra visits to the clinic.Item Correlation between 18F-FDG Positron-Emission Tomography 18F-FDG Uptake Levels at Diagnosis and Histopathologic and Immunohistochemical Factors in Patients with Breast Cancer(AVES, 2016-01-01) Ugurluer, Gamze; Yavuz, Sinan; Calikusu, Zuleyha; Seyrek, Ertugrul; Kibar, Mustafa; Serin, Meltem; Ersoz, Canan; Demircan, OrhanObjective: In this study, we aimed to determine the correlation between pretreatment-staging 18F-FDG total body positron-emission tomography/computed tomography (PET/CT) maximum standardized uptake value (SUVmax) levels and histopathologic and immunohistochemical predictive and prognostic factors in patients with breast cancer. Materials and Methods: One hundred thirty-nine women with breast cancer who were treated between 2009 and 2015 at our hospital and who had pretreatment-staging PET/CT were included in the study. SUVmax levels and histopathologic and immunohistochemical results were compared. Results: The median age was 48 years (range, 29-79 years). The mean tumor diameter was 33.4 mm (range, 7-120 mm). The histology was invasive ductal carcinoma in 80.6\% of the patients. It is accepted that lymph node involvement with metastatic tumor and the presence of distant metastasis are the most important prognostic factors. Accurate staging is important in determining prognosis and appropriate treatment. Positron emission tomography with computed tomography detectsmalignancies using 2-{[}18F]-fluoro-2-deoxy-d-glucose (18F-FDG PET CT) with high accuracy and they contribute to decisions regarding diagnosis, staging, recurrence, and treatment response. Here, we report a case of false positive metastatic mediastinal lymph nodes that were diagnosed by 18F-FDG PET CT in a 40-year-old breast cancer patient who had undergone preoperative evaluation. Right paratracheal, prevascular, aorticopulmonary, precarinal, subcarinal, hilar, and subhilar multiple conglomerated mediastinal lymph nodes were revealed in addition to left breast mass and axillary lymph nodes. Mediastinoscopy was performed with biopsy and pathology was reported as granulomatous lymphadenitis. In conclusion, any abnormal FDG accumulation in unusual lymph nodes must be evaluated carefully and confirmed histopathologically.Item Neuroprotective effects of erythropoietin against oxidant injury following brain irradiation: an experimental study(TERMEDIA PUBLISHING HOUSE LTD, 2016-01-01) Ugurluer, Gamze; Cebi, Aysegul; Mert, Handan; Mert, Nihat; Serin, Meltem; Erkal, Haldun SukruIntroduction: Radiation therapy (RT) is a major treatment modality, and the central nervous system is a dose-limiting organ in clinical RT. This experimental study aims to present the evaluation of the neuroprotective effects of erythropoietin (EPO) against oxidant injury following brain irradiation in rats. Material and methods: Forty Wistar rats were randomly assigned to four groups (n = 10 each). In group 1 the rats received no EPO and underwent sham RT. The rats in groups 2 and 3 received EPO. In group 2 rats underwent sham RT, while in group 3 rats received RT. The rats in group 4 received no EPO and underwent RT. Rats were irradiated using a Cobalt-60 teletherapy machine using a single fraction of 20 Gy covering the whole brain. Cervical dislocation euthanasia was performed. The nitrite and malondialdehyde (MDA) levels and the superoxide dismutase (SOD) and glutathione peroxidase (GSHPX) activities were evaluated in dissected brain tissues. Results: The nitrite and MDA levels were higher in the RT group (2.10 +/-0.62 ppm, 26.02 +/-2.16 nmol/mlItem Prostat Kanseri Rektum Metastazı(Acıbadem Mehmet Ali Aydınlar Üniversitesi, 2013-07-01) Uğurluer, Gamze; Arpacı, Taner; Çalıkuşu, Züleyha; Akbaş,Tuğana; Serin, MeltemÖZET Prostat kanseri erkeklerde sık görülen ve metastaz yapma riski yüksek olan kanserlerden biridir ancak rektum metastazı nadirdir. 75 yaşında erkek hasta 2008 yılında metastatik prostat kanseri (multiple kemik metastazı) tanısı almış. 2012 yılında rektal kanama nedeniyle başvuran ve anemi tanısı konan hastanın kolonoskopisinde rektumda polipoid kitle lezyonu saptanmış. Biyopsi yapılmış ve metastatik prostat adenokarsinomu olarak raporlanmış. Kliniğimize rektal kanama şikayeti ile başvuran hastaya palyatif radyoterapi uygulandı.Item Radiotherapy Induced Changes of Masticatory Muscles and Parotid Glands on MRI in Patients with Nasopharyngeal Carcinoma(Acıbadem Mehmet Ali Aydınlar Üniversitesi, 2019-04-04) Arpacı, Taner; Uğurluer, Gamze; Örekici, Gülhan; Akbaş, Tuğana; Serin, MeltemABSTRACT Objective: The aim of this study was to identify radiotherapy (RT) induced changes of masticatory muscles and parotid glands on magnetic resonance imaging (MRI) in patients with nasopharyngeal carcinoma (NPC). Patients and Methods: Thirty-one patients treated with RT for NPC between 2009-2016 in our instution were included in the study. MRI examinations performed before and after RT were reviewed retrospectively. Transverse diameters and signal intensities of masticatory muscles and parotid glands were evaluated on T2 weighted axial MR images. Normal (hypointense) signal was scored as 0, mild hyperintensity as 1 and severe hyperintensity as 2. Results:The mean interval between pre-RT MRI and last control MRI was 44 months (12-84 months). Reductions of transverse diameters were; right masseter (RM):2.32mm (15%), left masseter (LM):2.42mm (15.4%), right medial pterygoid (RMP):1.26mm (8.7%), left medial pterygoid (LMP):1.71mm (12%), right lateral pterygoid (RLP):1.35mm (9.6%), left lateral pterygoid (LLP):1.32mm (9.4%), right parotid gland (RP):8.22mm (26%), left parotid gland (LP):8.87mm (28%). T2 signal changes were; RM: mild 8 cases (26%), LM: mild 5 cases (16%), RMP: mild 5 cases (16%), severe 1 case (3.5%), LMP: mild 4 cases (13%), severe 2 cases (6.5%), RLP and LLP: mild 8 cases (26%), severe 3 cases (9.7%), RP: mild 10 cases (32%), severe 18 cases (58%), LP: mild 10 cases (32%), severe 17 cases (55%). Conclusion: Volume loss and hyperintensity were mostly seen in parotid glands. Masseter was the muscle with highest rate of volume loss and lateral pyterygoid showed the highest rate of hyperintensity in masticatory muscles.Item Serviks Kanserli Hastalarda PET/BT Tetkikinde Primer Tümörün FDG Tutulum Derecesi ile Lenf Nodu Metastazı Arasındaki İlişki(Acıbadem Mehmet Ali Aydınlar Üniversitesi, 2016-03-01) Uğurluer, Gamze; Atalar, Banu; Saran İkizler, Hayriye Aslı; Sözer, Nihat; Kibar, Mustafa; Serin, Meltem; Özyar, EnisÖzet Amaç: Bu çalışmada, serviks kanserli hastalarda Pozitron Emisyon Tomografisi/Bilgisayarlı Tomografi (PET/BT) tetkikinde primer tümörün FDG tutulum derecesi ile lenf nodu metastazı arasındaki ilişkinin saptanması amaçlandı. Gereç ve yöntem: Tedavi öncesi evreleme amacıyla PET/BT tetkiki yapılan 103 hasta çalışmaya dahil edildi. Primer tümörün mak simum standart uptake (SUVmaks) değeri, PET/BT tetkikinde ölçülen maksimum tümör çapı, pelvik ve paraaortik lenf nodlarında FDG tutulumu olup olmadığı ve FDG tutulumu olan lenf nodlarında SUVmaks değerleri her hasta için kaydedildi. SUVmaks değer lerinin lenf nodu metastazı ile ilişkisi araştırıldı. İstatistiksel analiz SPSS programı ile yapıldı. Bulgular: Ortanca yaş 56 idi (31-91 arası). SUVmaks ortalama değerleri primer tümör için 14,3±6,3 (3,9-34,2 arası) ve lenf nod ları için 8,6±3,9 (2,8-19,3 arası) idi. Lenf nodu tutulumu olan ve olmayan hastalarda SUVmaks ortalama değerleri sırasıyla 15,9 (4,1-34,2 arası) ve 11,9 (3,9-25,5 arası) idi (p <0,05). Medyan SUVmaks değerine (13,9) göre düşük ve yüksek olarak sınıflandırı lan gruplarda ortalama değerler sırasıyla 9,3 (3,9-13,4 arası) ve 18,9 (13,9-34,2 arası) idi. Düşük SUVmaks grubunda hastaların %46’sında, yüksek SUVmaks grubunda %70’inde lenf nodu tutulumu vardı (p <0,05). Tümör çapı ≤4 cm ve >4 cm olan hastalarda SUVmaks ortalama değerleri 13,1 (5,5-25,5 arası) ve 17,1 (7,7-34,2 arası) idi. Tümör çapı ≤4 cm olan hastaların %42’sinde ve >4 cm olan hastaların %66’sında lenf nodu tutulumu vardı. SUVmaks değerleri ve lenf nodu FDG tutulumu açısından iki grup arasın daki fark istatistiksel olarak anlamlıydı (p <0,05). Sonuç: Serviks kanserli hastalarda primer tümörde yüksek SUVmaks değerleri lenf nodu metastazı açısından yüksek riski göstere bilir ve hastaların prognozunu ve tedavi yaklaşımının seçilmesini etkileyebilir.
