Item Atrial fibrillation designation with micro-Raman spectroscopy and scanning acoustic microscope(NATURE PORTFOLIO, 2022-01-01) Parlatan, Ugur; Parlatan, Seyma; Sen, Kubra; Kecoglu, Ibrahim; Ulukan, Mustafa Ozer; Karakaya, Atalay; Erkanli, Korhan; Turkoglu, Halil; Ugurlucan, Murat; Unlu, Mehmet Burcin; Tanoren, BukemAtrial fibrillation (AF) is diagnosed with the electrocardiogram, which is the gold standard in clinics. However, sufficient arrhythmia monitoring takes a long time, and many of the tests are made in only a few seconds, which can lead arrhythmia to be missed. Here, we propose a combined method to detect the effects of AF on atrial tissue. We characterize tissues obtained from patients with or without AF by scanning acoustic microscopy (SAM) and by Raman spectroscopy (RS) to construct a mechano-chemical profile. We classify the Raman spectral measurements of the tissue samples with an unsupervised clustering method, k-means and compare their chemical properties. Besides, we utilize scanning acoustic microscopy to compare and determine differences in acoustic impedance maps of the groups. We compared the clinical outcomes with our findings using a neural network classification for Raman measurements and ANOVA for SAM measurements. Consequently, we show that the stiffness profiles of the tissues, corresponding to the patients with chronic AF, without AF or who experienced postoperative AF, are in agreement with the lipid-collagen profiles obtained by the Raman spectral characterization.Item Carotid artery plaque structure in the context of symptomatic/asymptomatic nature of carotid artery stenosis(CZECH SOC CARDIOLOGY \& CZECH SOC CARDIOVASCULAR SURGERY, 2021-01-01) Oztas, Didem Melis; Bilen, Bukem Tanoren; Sener, Leyla Turker; Beyaz, Metin Onur; Ulukan, Mustafa Ozer; Unal, Orcun; Unlu, Mehmet Burcin; Reis, Paulo Eduardo Ocke; Ugurlucan, MuratObjective: In this paper, we examined the relationship between atherosclerotic plaque characteristics and symptomatic carotid artery disease. Material and methods: Twenty-two patients who underwent carotid endarterectomy were enrolled into the study. Thirteen patients were male and remaining 9 patients were female. The mean age was 60.2 +/- 9.6 years. The carotid artery stenosis ranged between 75\% and 99\%. Eight patients were asymptomatic and remaining 14 patients were symptomatic. Ten patients suffered stroke. One of them had recurrent transient ischemic attacks, 4 patients had dizziness, and one of them had amaurosis fugax. The extracted plaques were evaluated by scanning acoustic microscopy (SAM) and micro-computerized tomography (micro-CT). Results: Smaller acoustic impedance values that reveal collagen-rich plaque formation were detected in patients who had stroke, while higher acoustic impedance values that reveal calcium-rich plaque formation were detected in the asymptomatic patients' plaques. Conclusions: Determination of plaque characteristics with modern radiographic techniques, rather than relying on the degree of stenosis in patients with carotid artery disease, may be helpful to discriminate patients requiring intervention.Item DETERMINATION OF LOWER EXTREMITY PERIPHERAL ARTERY DISEASE IN PATIENTS UNDERGOING CAROTID ENDARTERECTOMY(ISTANBUL UNIV, FAC MEDICINE, PUBL OFF, 2020-01-01) Onalan, Mehmet Akif; Bahseliyev, Siraslan; Beyaz, Metin Onur; Oztas, Didem Melis; Onal, Yilmaz; Ugurlucan, Murat; Tireli, EminObjective: Atherosclerotic vascular disease is a systemic pathology. The aim of our study was to determine the frequency of lower extremity peripheral artery disease (PAD) in patients who underwent carotid endarterectomy (CEA) and to identify risk factors. Material and Method: The study included 65 patients who underwent carotid endarterectomy in Istanbul Medical Faculty, Department of Cardiovascular Surgery between January 2011 and January 2015. The medical history, physical examination, ankle brachial index and lower extremity duplex ultrasonography results were evaluated and the patients were examined for peripheral artery disease. Results: Among 65 patients, 47 of them were male and 18 of them were female. The patients' ages ranged from 48 to 88 years, with an average of 66.26 +/- 8.84 years. In our study, being above the age of 65 (ODDS ratio (OR) 4.65, 95\% CI: 1.61-13.4), male gender (OR 6.75, 95\% CI: 1.71-26.50), smoking (OR 10.50, 95\% CI: 2.15-51.13), the family history (OR 023, 95\% CI: 0.06-0,80), the presence of chronic renal failure (OR 0.41, 95\% CI: 0.30-0.56) were found to increase peripheral artery disease in patients who underwent carotid endarterectomy statistically significant. In cases with low HDL levels, the frequency of peripheral artery disease was found increased. In our study, peripheral artery disease was detected in 30 (46.2\%) of 65 patients who underwent carotid endarterectomy operation. Conclusion: Due to systemic involvement of atherosclerosis, lower extremity peripheral artery disease should be considered in patients who underwent carotid endarterectomy operation, and patients should be evaluated accordingly.Item Evaluation of chronic inflammation in the aetiology of venous insufficiency by investigating cytomegalovirus DNA(TERMEDIA PUBLISHING HOUSE LTD, 2022-01-01) Beyaz, Metin O.; Bozkurt, Emre; Onalan, Mehmet A.; Oztas, Didem M.; Meric, Mert; Ulukan, Mustafa Ozer; Coskun, Ugur; Ugurlucan, MuratIntroduction: Lower extremity venous insufficiency is a significant health problem with economic and sociological consequences, lowering the quality of life, and sometimes leading to serious complications. The aim of this study is to evaluate the cytomegalovirus (CMV) effect on chronic inflammation in the aetiology of chronic venous insufficiency. Material and methods: Between November 2017 and August 2018, 468 patients who underwent radio-frequency ablation therapy and phlebectomy were included in the study. PCR analyses for CMV DNA were performed on the venous tissue samples. Patients with post-thrombotic syndrome were excluded from the study. After ethical approval, the relationship between the presence of CMV DNA, gender, body mass index, and bilaterality of chronic venous insufficiency were investigated. Results: When the relationship between CMV DNA and gender or body mass index was examined, a significant relationship was not detected. But when the patients with bilateral chronic venous insufficiency and patients with unilateral chronic venous insufficiency were compared regarding CMV DNA positivity, the patients with bilateral chronic venous insufficiency had significantly higher CMV DNA positivity (p = 0.002). Also, the incidence of venous ulcers in the CMV DNA exposed group was significantly higher. Conclusions: In the literature there are many studies showing that CMV triggers atherosclerosis, but there is no study in which CMV directly produces chronic venous insufficiency. The high rates of positivity suggest that CMV, which is the basis of chronic inflammation, may be a significant factor in the aetiology of chronic venous insufficiency.
