Case Report; Laparoscopy in the Management of Internal Herniation of Adnexa

Abstract Broad ligament herniation is defined as protrusion of viscera through the pelvic peritoneum. It accounts 5% of internal hernias. We report, a 17 year-old woman presenting with a 5cm adnexal anechoic cyst and episodic pelvic pain who has no remarkable medical history. After 8 hours of follow-up, clinical picture persisted and evaluation with laparoscopy. It was noted that right adnexa with 5cm cyst was herniated through broad ligament intraoperatively. Ovarian cyst was excised and hernia was repaired laparoscopically. Clinical findings were resolved after surgery and patient discharged the day after surgery. In conclusion, adnexal hernia through broad ligament should be thought in differential diagnosis of female acute pelvic pain. Laparoscopy is an effective procedure for the diagnosis and treatment of adnexal hernia.Abstract Broad ligament herniation is defined as protrusion of viscera through the pelvic peritoneum. Corneal Thickness and Topography Indices after Collagen Cross-linking for Keratoconus: 18 Months Follow-up and Literature Review

ABSTRACT Objective: To evaluate the changes in corneal topography indices and corneal thickness after cross-linking treatment for keratoconus. Study Design: Retrospective Observational Study Patients and Methods: The data of patients who underwent corneal collagen cross-linking treatment for keratoconus were retrieved. Central corneal thickness (CCT), maximum and mean keratometry (Kmax, Kmean) values and topography indices: index of surface variance (ISV), index of vertical asymmetry (IVA), keratoconus index (KI), central keratoconus index (CKI), minimum radius of curvature (Rmin), index of height asymmetry (IHA), and index of height decentration (IHD) were obtained from Pentacam and Wavelight-Allegretto Wave Topolyzer outputs. Follow-up was 18 months. Results: The study comprised of 15 eyes of 10 patients with keratoconus. Central corneal thickness had decreased significantly 1 month after the procedure (p=0.003) but there were no statistically significant changes between the baseline values and last visits (p>0.05). Kmax and Kmean values were not statistically different from the preoperative values and at the last visits (p>0.05). Almost all of the corneal topography indices improved significantly after postoperative 6th month (p<0.05). Conclusion: There were improvements in topography indices during 18-month follow-up period, suggesting that the cornea becomes more optically regular and symmetrical after cross-linking.
Diagnostic Role of sCD14-Subtype as a Sepsis Biomarker in Febrile Neutropenic Pediatric Oncology Patients

ABSTRACT Introduction and Objective: Febrile neutropenia (FN) is one of the most common complications of cancer chemotherapy and requires urgent treatment. Upon diagnosis of childhood FN, culture samples should be obtained and a broad spectrum antibiotherapy should be administered immediately. However, bacterial infections are not the only cause of fever in neutropenic pediatric patients; chemotherapeutics, the disease itself, as well as viral and fungal agents can also be the cause of fever. Various biomarkers have been studied to differentiate the cause of fever to date. Infections are mostly lethal in neutropenic patients, early stage differential biomarkers are, therefore, of utmost importance. It is the aim of this study to assess the potential of presepsin as an additional diagnostic tool for the detection of bacteremia/sepsis in childhood Febrile Neutropenia (FN) patients. Methods: Twenty-four pediatric patients, with a total 29 febrile neutropenic episodes were enrolled in this study. One patient with 3 FN episodes, three patients with 2 FN episodes were admitted to our clinic during the study. Patients were classified into bacteremia/sepsis and fever without origin groups. Serum samples were collected after confirmation of FN and analyzed for presepsin, c-reactive protein (CRP) and procalcitonin (PCT) according to instructions of manufacturers. Results: Biomarkers failed to display a discriminative value between bacteremia/sepsis and fever without origin groups, whereas presepsin was found to be an indicator of the presence of bacterial growth in hemoculture and was shown to have a potential diagnostic value. Conclusion: Presepsin might be used as an additional diagnostic tool for the detection of bacteremia/sepsis in childhood FN patients.
Does the Screw Orientation Effects the Stability of Femoral Neck Fracture?: A Finite Element Analysis

ABSTRACT Objectives: The incidence of femoral neck fractures in younger-age people increases due to high-energy trauma. To avoid complications, stable internal fixation is essential. The most commonly used implants for vertically oriented femoral neck fractures are the cannulated and dynamic hip screws. In our study, we compared differently oriented cannulated screw configurations and a standard DHS for fixation of femoral neck fracture. Materials and Methods: A finite element-based collum femoris fracture was created and was fixed using four differently oriented triangular screw configurations and a dynamic hip screw. The loads were applied using a commercially available software package. Results: Centrally oriented cannulated screw configuration had the most compression and compression stress on the fracture side, and it had the highest stress values on the implant. Conclusion: We recommend the use of centrally oriented triangular cannulated screws for femoral neck fracture fixation. With this construct, more compression and compression pressure can be obtained, which can result in the early healing of the fracture. However, it must be kept in mind that this construct exerts more stress on the implant, which could be a reason for implant failure.
Hemşirelerin Yönetsel Kararlara Katılımlarının İş Yaşam Kalitesine Etkisi

ÖZET Amaç: Hastanelerde hemşirelerin iş yaşamı kalitesinin arttırılması, hasta bakımının kalitesini ve hasta memnuniyetini de arttırır. Bu çalışmada, hemşirelerin yönetsel kararlara katılımının iş yaşamı kalitesi üzerine etkileri araştırılmıştır. Çalışma Planı: Tanımlayıcı ve kesitsel olarak gerçekleştirilen çalışmanın örneklemini özel bir hastanede çalışan 180 hemşire oluşturdu. Çalışmanın verileri, Tanıtıcı Bilgiler Formu, Kararlara Katılım Ölçeği (KKÖ) ve Hemşirelerin İş Yaşam Kalitesi Ölçeği (HİYKÖ) ile değerlendirildi. Verilerin analizinde tanımlayıcı istatistiklerin yanı sıra t-testi, Mann- Whitney testi ve Kruskal Wallis varyans analizi kullanıldı. Veriler %95 güven aralığında değerlendirildi. Bulgular: Çalışmaya katılan hemşirelerin %26,7’si (n=48) 25 yaşın altında idi, %72,2’si (n=130) lisans mezunu olup, %85’i (n=153) servis hemşiresi olarak çalışıyordu. Yaş, eğitim düzeyi, devam eden bir öğretim programının varlığı, kurs ve sertifika programlarına katılım, pozisyon değişikliği ve çalışma süresinin iş yaşamı kalitesi üzerine etkisi istatistiksel olarak anlamlı bulundu (p<0.05). Hemşirelerin KKÖ puanları mevcut durum için 1,78±0.52; istenen durum için ise 2,6±0.63 olarak belirlendi (p<0.001). Sonuç: Hemşirelerin iş yaşamı kalitesini belirleyen faktörlerin iyileştirilmesine yönelik kurumsal düzenlemeler ve yönetsel kararlara katılım yoluyla hemşirelerin güçlendirilmesi, iş yaşamı kalitesini geliştirmeye katkı sağlayacaktır.
Kompleks Kalp Anomalisi ile Birlikte Protein C Eksikliği Bulunan Çocukta Aşamalı Tam Düzeltme Operasyonu

ÖZET Kalp cerrahisi uygulanacak, koagulopati tanısı olan hastalar, yüksek oranda kanama ve tromboz riski ile karşı karşıya kalabilirler. Bu çalışmada kompleks kalp anomalisi ile birlikte Protein C eksikliği tanısı bulunan ve 4 yaşında, aşamalı Rastelli operasyonu uyguladığımız hastamız sunulmaktadır. Protein C eksikliği tanısı, hastamız 1.5 yaşında iken modifiye-BT şant uygulanmasının ardından erken dönem şant trombozu oluşumu nedeniyle yapılan tetkikler sonucunda konulmuştu. Şant ameliyatı sonrasında 2.5 yıl antikoagülan ve antiagregan tedaviyle şantı açık olarak izlenmiş ve 4 yaşında başarılı Rastelli operasyonu uygulanmıştır. Olgumuzu paylaşmaktaki amacımız kalp cerrahisi sonrası tromboembolik olaylara yol açabilen predispozan faktörlerin önemine dikkat çekmektir
Nadir Bir Olgu ve Hemşirelik Bakımı: Pulmoner Alveoler Proteinozis

ÖZET Pulmoner alveoler proteinozis (PAP), alveollerde lipoproteinoz materyal birikimi ile karakterize nadir görülen bir akciğer hastalığıdır. Hastalık ilk olarak 1958’de tanımlanmıştır. Literatüre 500 civarında olgu bildirilmiştir. En sık rastlanan semptomlar nefes darlığı, yorgunluk hissi ve kuru öksürüktür. Hastalığın konjenital, sekonder ve idiyopatik olmak üzere üç formu vardır. En sık %90 oranla idiyopatik formu görülmektedir. Hastalığın mortalitesi %10 civarındadır. Kronik bir hastalık grubuna giren pulmoner alveoler proteinozis, kişinin günlük yaşam aktivitelerinde değişikliği yol açabilmektedir. Takip ve tedavisi sırasında hemşirelik bakım uygulamaları ve hasta eğitimi önemli yer tutmaktadır. Hastalık uzun yıllar tedavi gerektirdiğinden tedavi ve bakımda profesyonel ekip birlikteliği önemli yer tutmaktadır. Hemşirenin rolleri olan tedavi, bakım ve eğitim verme bu hastalıkta önemli bir yer tutar. Bu açıdan bu vaka literatür için önemlidir.
