Histopathologically, epidermal invagination showing luminal epithelial cells with decapitation secretion is diagnostic. Herein, we describe an unusual late-onset case of syringocystadenoma papilliferum mimicking basal cell carcinoma on the face.Item Adult Type Xanthogranuloma: A Case Report(GALENOS YAYINCILIK, 2011-01-01) Sezer, Engin; Erkek, Emel; Sahin, Sedef; Cetin, Emel DikiciogluCutaneous xanthogranuloma is a dermatosis, clinically presenting as papules, nodules or plaques and histopathologically characterized by diffuse dermal infiltration of epithelioid cells with vesicular nuclei and conspicuous cytoplasm showing focal xanthomatous changes, and a mixed inflammation including eosinophils, neutrophils and lymphocytes. Although the disease is commonly observed in childhood, adult cases have also been reported and may cause confusion with various disorders histopathologically, including malignant melanoma. Herein, we report a case of adult type xanthogranuloma with reference to the literature.Item Childhood alopecia areata: A study of 89 patients(DERI ZUHREVI HASTALIKLAR DERNEGI, 2014-01-01) Dogan, Sibel; Ersoy-Evans, Sibel; Gonc, Nazli; Sahin, SedefBackground and Design: Alopecia areata (AA) is a frequent type of acquired hair loss with a sudden onset. The aim of this study was to investigate the epidemiological and clinical features of AA and diseases associated with AA in the Turkish pediatric population. Materials and Methods: We retrospectively evaluated the medical data of patients under 16 years of age who were admitted to the pediatric dermatology outpatient clinic at Hacettepe University Hospital between 2006 and 2011 with a diagnosis of AA, alopecia totalis (AT) and alopecia universalis (AU). Results: A total of 89 patients (34 females and 55 males), were included in this study. The mean age of the subjects was 9.8 +/- 3.4 years (range:18 months-16 years). 9\% (n=8) patients had other autoimmune diseases: Hashimoto's thyroiditis in 5, vitiligo in 1, juvenile rheumatoid arthiritis (JRA) in 1 and morphea in 1 patient. AA was located on the scalp in 86.5\% (n=77) of the patients, 7.9\% (n=7) of the patients had AA on the scalp together with eyelashes, eyebrows and the body. AT was noted in 1, AU was noted in 4 patients. Severity of hair loss was <25\% in 94.3\% (n=84) of the patients according to the involved area. Materials and Methods: The patients referred to dermatology outpatient clinics of Acibadem Group Hospitals in Istanbul between the time intervals 2019 March 1st and August 31st (Group G1) and 2020 March 1st and August 31st (Group G2) were included in this study. G1 and G2 groups were compared in terms of age, gender, mean duration of complaints, the most common five anatomic regions and the most common five dermatoses. Results: Referral rate after Covid-19 pandemic was decreased by 40\%. Face (p=.0013, OR= 1.20, 95\% CI=1.07-1.34), scalp (p=.02, OR=1.23, 95\% CI=1.02-1.48) and hand (p=.04, 95\% OR=1.24, 95\% CI=1.00-1.54) were more common locations in 2020 (group G2) than in 2019 (G2 group). The most common five dermatoses were contact dermatitis (8.3\%), acne (8,1\%), wart (7,0\%), seborrheic dermatitis (5,7\%) and pyoderma (5,5\%) in G1 (2019), while the order was contact dermatitis (10,6\%), acne (7,5\%), seborrheic dermatitis (6,5\%), wart (6,4\%) and pyoderma (4,9\%) in G2 (2020). Contact dermatitis showed a significant increase after pandemic (p=.0017, OR=1.29, 95\% CI=1.10-1.51). In 2020, psoriasis and lichen planus showed a two-fold increases (P <.001, OR=2.00, 95\% CI=1.43-2.79 and p=.03, OR=1.92, 95\% CI=1.09-3.38, respectively), whereas frequency of molluscum contagiosum decreased significantly (p=.03, OR=0.49, 95\% CI=0.26-0.92). Its clinical presentation is usually initially insidious and it often remains unrecognised because of its non-specific clinical presentation, which can cause delays in treatment and high mortality. The most common physical symptoms are cutaneous. We present a lethal case of CES to increase the awareness about this serious condition.Item Painless dells on the cheek(MEDKNOW PUBLICATIONS \& MEDIA PVT LTD, 2013-01-01) Duman, Deniz; Durmaz, Emel Ozturk; Cetin, Emel A.; Yazar, Sukru; Sahin, SedefItem Papular elastorrhexis: clinical perspectives(DOVE MEDICAL PRESS LTD, 2018-01-01) Sezer, Engin; Durmaz, Emel Ozturk; Sahin, SedefFirst described by Bordas in 1987, papular elastorrhexis (PE) is a rare elastic fiber disorder of the skin characterized by multiple, discrete, asymptomatic, firm, nonfollicular, monomorphous, 1-5 mm, circumscribed, hypopigmented, oval to round papules, symmetrically distributed on the chest, abdomen, back, shoulders, arms, and thighs. The onset of the condition is usually in the first or second decade of life. PE appears to be an exceedingly rare entity, with 33 cases reported in the literature until now. However, the disorder might be underestimated probably because of its subtlety, asymptomatic course, and benign nature of clinical alterations, which can easily be confused with other dermatoses such as acne scars. Clinical and histopathological differential diagnosis of PE is broad and includes papular acne scars, eruptive collagenoma, disseminated lenticular dermatofibrosis (as a component of Buschke-Ollendorff syndrome), white fibrous papulosis of the neck, pseudoxanthoma elasticum, pseudoxanthoma elasticum-like papillary dermal elastolysis, middermal elastolysis, and perifollicular elastolysis. Treatment of PE is a matter of debate and no reliable curative option exists.Item Retrospective analysis of melanoma patients who had undergone PET-CT examination(DERI ZUHREVI HASTALIKLAR DERNEGI, 2013-01-01) Bakar, Ozgur; Vardareli, Erkan; Sahin, Sedef; Yuksel, Selcen; Aksoy, TamerBackground and Design: Pet positivity was found in 21 (37.5\%) the incidence of melanoma is increasing significantly in Turkey and in the whole world. Thus, Positron Emission Tomography-Computed Tomography (PET-CT), which is being reimbursed by the government for the documentation of metastases of clinically suspicious patients with stages 1 b and higher, is becoming widely used. This study aims to analyze, the histopathological features of the tumors and the clinical and demographic data on melanoma patients who had undergone PET-CT examination for the screening of metastases. Materials and Methods: We collected data on 57 melanoma patients who had undergone PET-CT evaluation in the Nuclear Medicine Department between 2004 and 2011. On the basis of PET-CT positivity, we retrospectively analyzed SUV (Standard uptake value), demographic data, Breslow thickness, primary tumor location, presence of histopathologically documented ulceration, perineural, vascular and lymphatic invasion, and metastases detected by other screening methods as well as the proposed treatment modalities. Results: PET-CT positivity was found in 22 (39\%) patients. The mean Breslow thickness was 5.12 +/- 6.29mm., 1 patient (\%7) with <= 1mm, 5 (36\%) with 2-4mm and 8 patients (57\%) with >4mm of Breslow thickness had distant metastases. Statistically significant correlation was found between Breslow thickness and positive PET-CT findings. The distribution of the primary tumor site was 35.3\%left-sided, 25.5\%right-sided and 39.2\%on the midline. All patients with scalp involvement had distant metastases. There was no statistically significant correlation of PET-CT positivity with histopathologically proven ulceration and perineural, vascular and lymphatic invasion. Thirty-one percent positive correlation was found between Breslow thickness and SUV. Forty-five percent of PET-CT positive patients had metastases documented with other screening methods as well. All but 2 patients with distant metastases had undergone wide tumor resection and adjuvant chemotherapy or radiotherapy. The pathogenesis has been attributed to abnormal and delayed keratinization. Although affected patients present with typical lesions, the disorder is not well-known by dermatologists. In this report, we describe two patients with terra firma-forme dermatosis in the setting of xerosis cutis and atopic dermatitis. From a clinical point of view, we lay emphasis on its unique expression and diagnosis/treatment. From a histological perspective, we highlight its resemblance to dermatosis neglecta and speculate on the role of `neglect' in a patient with seemingly adequate hygiene. According to the proposed guidelines for MF, skin-directed therapies are the treatment of choice for patients with limited stage disease. We present a case of early-stage MF, who progressed to tumor-stage MF during the postpartum period, showing a solitary ulcerated tumor on the vulva, which was successfully treated with local response-based, low-dose radiotherapy.
