Item Early Compaction Might Be a Parameter to Determine Good Quality Embryos and Day of Embryo Transfer in Patients Undergoing Intracytoplasmic Sperm Injection(CUREUS INC, 2022-01-01) Ozturk, Senem Aslan; Cincik, Mehmet; Cakil, Yaprak Donmez; Sayan, Sena; Selam, BelginIntroduction: Compaction is the first event in embryo morphogenesis. Blastocyst transfer on day five or six has been widely performed in the last decade. We investigated the clinical value of early compaction on day three for evaluation of the transferred embryo quality and pregnancy. Methods: Four hundred patients with female factor infertility and 776 fresh embryo transfers were included. Two groups were formed: Early compaction group had embryo transfer with at least one day-three embryo exhibiting early compaction. Transferred embryos without early compaction comprised the control group. Embryo transfer was performed on day three or five after the assessment of embryo compaction by a time-lapse technology system. Each patient underwent only a single cycle of embryo transfer. We analyzed fertilization, pregnancy, and live birth rates. Results: We detected significantly higher numbers of the retrieved oocytes, metaphase II (MII) oocytes, and fertilized oocytes in the early compaction group. Moreover, the transfer of the early compacting embryos on day three resulted in higher pregnancy and live birth rates. Conclusion: Our data suggest that early compaction might be a factor to determine good quality embryos and embryo transfer day.Item Endometrial Polyp in Early Pregnancy Ending with Term Labor(Acıbadem Mehmet Ali Aydınlar Üniversitesi, 2020-01-01) Selam, Belgin; Danışman, Nuri; Ekmen, Süheyla; Bircan, CeydaABSTRACT Endometrial polyps may be detected by transvaginal ultrasonography (TVS) in early pregnancy. These pregnancies may end with fetal demise. There are not enough data in literature defining term labor in pregnancy with endometrial polyp. The current case describes in detail, the progress of a pregnancy with endometrial polyp including fetal growth, regression of polyp, assessment of placental pathology and postpartum follow-up. Giant endometrial polyp protruding into the uterine cavity adjacent to the gestational sac was detected by TVS in a 31 year-old-primigravida. She delivered a healthy baby and the polyp regressed completely by the end of the pregnancy. Polyps detected in early pregnancy may regress without any effect on the fetal demise or placental insufficiency. Close antenatal follow-up is advised for the prognosis of these pregnancies.Item Luteal Faz Desteği Başlama Zamanlarının Randomize Karşılaştırılması(Acıbadem Mehmet Ali Aydınlar Üniversitesi, 2010-10-01) Demirel, L. Cem; Selam, Belgin; Ergin, Tolga; Lembet, Arda; Kaya, Deniz Gökalp; Öztürk, Harika BodurÖZET Randomize kontrollü çalışmamızın amacı luteal faz desteğinin başlamasında farklı zaman sürelerinin in vitro fertilizasyon (IVF) siklüslerinde klinik sonuçlar üzerine etkisini araştırmaktır. Ardışık 180 IVF siklüsünün 92’si A grubu, 88’i B grubu olarak düzenlendi. Grup A’da luteal faz vajinal mikronize progesteron, oosit toplanmasından (OPU) hemen sonra, grup B’de ise OPU sonrası 48. saatte başlandı. Her iki grupta da transfer başına klinik gebelik oranı ve embriyo başına implantasyon oranı benzer bulundu (sırasıyla %34.ve %35.4, p:1.00; %13.8 ve %15.3, p:0.59). Çalışma gruplarındaki abortus oranları arasında fark izlenmedi. IVF siklüslerinde progesteron desteğinin OPU günü ya da 48 saat sonra başlanması klinik sonuçları değiştirmemektedir.Item Rescue IVM of Denuded GV- and MI-Stage Oocytes of Premenopausal Rats with Oncostatin M, Insulin-like Growth Factor I, and Growth Hormone(MDPI, 2022-01-01) Akdemir, Yesim; Donmez Cakil, Yaprak; Selam, Belgin; Sitar, Mustafa Erinc; Cincik, MehmetImmature oocytes are retrieved and matured through in vitro maturation (IVM). Maturation, fertilization rates, and embryo development via IVM are all lower than those found in vitro fertilization (IVF) cycles. We investigated the effects of oncostatin M (OSM), insulin-like growth factor-1 (IGF-I), and growth hormone (GH) in rescue IVM. A total of 111 germinal vesicle (GV) and 17 metaphase I (MI) oocytes were obtained after conventional IVF from 28 female Wistar albino rats. Denuded immature oocytes were cultured in maturation medium supplemented with OSM, IGF-1, or GH. The quantities of metaphase II (MII) oocytes matured from the GV stage were 17 of 30 (56.6\%), 15 of 28 (53.5\%), 10 of 30 (33.3\%), and 7 of 23 (30.3\%), in control, OSM, IGF-I, and GH groups, respectively. Maturation rates in control and OSM groups were higher than those in IGF-I and GH groups (p = 0.001). The quantities of MII oocytes matured from MI stage were 7 of 7 (100\%), 4 of 4 (100\%), 1 of 1 (100\%), and 1 of 5 (20\%) in control, OSM, IGF-I, and GH groups, respectively. Maturation rates from MI to MII stages in control, OSM, and IGF-I groups were higher than those in the GH group (p = 0.004). Acceptable maturation rates are observed with OSM in rat oocytes in rescue IVM.Item Schwannoma of the clitoris: a case report(IMR PRESS, 2021-01-01) Kocak, Alper; Selam, Belgin; Akar, Eylem; Erkanli, SerkanSchwannoma (neurilemoma or neurinoma) is a tumor that develops from the Schwann cells in the myelin sheath of the peripheral nerves. It is most commonly observed on the head, neck, upper and lower extremities, posterior mediastinum and retroperitoneum. Schwannomas are rarely found in the external female genitalia. To the best of our knowledge, only 6 cases of clitoral schwannoma have been reported in medical literature. We currently report the case of a 46-year-old woman who was presenting with a 3 X 2 cm, painless mass in her clitoris for the past two years. She has no history of neurofibromatosis. Surgical excision of the mass was undertaken. Histopathology and immunohistochemistry revealed schwannoma of the clitoris. No evidence of local recurrence was noted after 6 months of follow-up. We are reporting the current patient because of the rare location of schwannoma. In the present case report, clinical features, treatment guidelines, diagnostic features and the immunohistochemical characteristics of this tumor are reviewed.Item Sezaryen Doğumda İnsidental Omental Kist Hidatik: Olgu Sunumu(Acıbadem Mehmet Ali Aydınlar Üniversitesi, 2011-01-01) Öztürk, Harika Bodur; Selam, Belgin; Bilgi, SelçukÖZET Kist hidatik gebelik sırasında nadir rastlanan, tüm organları tutabilen parazitik bir patolojidir. Otuz altı yaşında ilk gebeliği bulunan hasta ya 39. gebelik haftasında maternal istek nedeniyle elektif sezaryen do ğum planlandı. Hastanın özgeçmişinde, 14 yıl önce geçirilmiş karaci ğer kist hidatik operasyonu bulunmaktaydı. Kanama kontrolü sırasın da, omentumda düzgün yüzeyli beyaz renkli, semikistik kitle palpe edil di. Kistektomi uygulandı. Histopatolojik inceleme sonucu kist hidatik olarak bildirildi. Gebelik sırasında hücresel bağışıklığın azalmasıyla hastalığın tekrar lama veya hızlı ilerleme riski bulunmaktadır. Hastalığın gebelikte na dir görülmesi nedeniyle, gebelik sırasında nüks oranları bilinmemekte dir ve tedavi konusunda fikir birliği yoktur. Gebelik sırasında tanı konu lan olgularda, cerrahi ile kistin eksizyonu önerilen ve tercih edilen te davi yöntemidir.Item Sperm Selection and Embryo Development: A Comparison of the Density Gradient Centrifugation and Microfluidic Chip Sperm Preparation Methods in Patients with Astheno-Teratozoospermia(MDPI, 2021-01-01) Guler, Cagla; Melil, Sureyya; Ozekici, Umit; Donmez Cakil, Yaprak; Selam, Belgin; Cincik, MehmetIn recent years, microfluidic chip-based sperm sorting has emerged as an alternative tool to centrifugation-based conventional techniques for in vitro fertilization. This prospective study aims to compare the effects of density gradient centrifugation and microfluidic chip sperm preparation methods on embryo development in patient populations with astheno-teratozoospermia. In the study, the semen samples of the patients were divided into two groups for preparation with either the microfluidic or density gradient methods. Selected spermatozoa were then used to fertilize mature sibling oocytes and the semen parameters and embryo development on days 3 and 5 were assessed. While the density gradient group was associated with a higher sperm concentration, motility (progressive and total) was significantly higher in the microfluidic chip group. No significant differences were observed in the fertilization rates or grade 1 (G1) and grade 2 (G2) proportions of the third-day embryos. Furthermore, while the proportions of the poor, fair and good blastocysts on day 5 did not differ significantly, excellent blastocysts (indicating high-quality embryos) were observed in a significantly higher proportion of the microfluidic chip group. Meticulous evaluation is important for the differential diagnosis of severe and progressive hip and/or groin pain in pregnant patients. MRI is a valuable and safe technique for demonstrating bone marrow edema and skeletal abnormalities during pregnancy. Avoidance of vaginal delivery and non-weight bearing measures are essential in order to prevent complications such as hip fractures related to transient osteoporosis of pregnancy. We present the diagnostic evaluation and treatment of an uncommon case of transient osteoporosis of pregnancy with resolution of symptoms and postpartum.
