Item Carotid artery plaque structure in the context of symptomatic/asymptomatic nature of carotid artery stenosis(CZECH SOC CARDIOLOGY \& CZECH SOC CARDIOVASCULAR SURGERY, 2021-01-01) Oztas, Didem Melis; Bilen, Bukem Tanoren; Sener, Leyla Turker; Beyaz, Metin Onur; Ulukan, Mustafa Ozer; Unal, Orcun; Unlu, Mehmet Burcin; Reis, Paulo Eduardo Ocke; Ugurlucan, MuratObjective: In this paper, we examined the relationship between atherosclerotic plaque characteristics and symptomatic carotid artery disease. Material and methods: Twenty-two patients who underwent carotid endarterectomy were enrolled into the study. Thirteen patients were male and remaining 9 patients were female. The mean age was 60.2 +/- 9.6 years. The carotid artery stenosis ranged between 75\% and 99\%. Eight patients were asymptomatic and remaining 14 patients were symptomatic. Ten patients suffered stroke. One of them had recurrent transient ischemic attacks, 4 patients had dizziness, and one of them had amaurosis fugax. The extracted plaques were evaluated by scanning acoustic microscopy (SAM) and micro-computerized tomography (micro-CT). Results: Smaller acoustic impedance values that reveal collagen-rich plaque formation were detected in patients who had stroke, while higher acoustic impedance values that reveal calcium-rich plaque formation were detected in the asymptomatic patients' plaques. Conclusions: Determination of plaque characteristics with modern radiographic techniques, rather than relying on the degree of stenosis in patients with carotid artery disease, may be helpful to discriminate patients requiring intervention.Item DETERMINATION OF LOWER EXTREMITY PERIPHERAL ARTERY DISEASE IN PATIENTS UNDERGOING CAROTID ENDARTERECTOMY(ISTANBUL UNIV, FAC MEDICINE, PUBL OFF, 2020-01-01) Onalan, Mehmet Akif; Bahseliyev, Siraslan; Beyaz, Metin Onur; Oztas, Didem Melis; Onal, Yilmaz; Ugurlucan, Murat; Tireli, EminObjective: Atherosclerotic vascular disease is a systemic pathology. The aim of our study was to determine the frequency of lower extremity peripheral artery disease (PAD) in patients who underwent carotid endarterectomy (CEA) and to identify risk factors. Material and Method: The study included 65 patients who underwent carotid endarterectomy in Istanbul Medical Faculty, Department of Cardiovascular Surgery between January 2011 and January 2015. The medical history, physical examination, ankle brachial index and lower extremity duplex ultrasonography results were evaluated and the patients were examined for peripheral artery disease. Results: Among 65 patients, 47 of them were male and 18 of them were female. The patients' ages ranged from 48 to 88 years, with an average of 66.26 +/- 8.84 years. In our study, being above the age of 65 (ODDS ratio (OR) 4.65, 95\% CI: 1.61-13.4), male gender (OR 6.75, 95\% CI: 1.71-26.50), smoking (OR 10.50, 95\% CI: 2.15-51.13), the family history (OR 023, 95\% CI: 0.06-0,80), the presence of chronic renal failure (OR 0.41, 95\% CI: 0.30-0.56) were found to increase peripheral artery disease in patients who underwent carotid endarterectomy statistically significant. In cases with low HDL levels, the frequency of peripheral artery disease was found increased. In our study, peripheral artery disease was detected in 30 (46.2\%) of 65 patients who underwent carotid endarterectomy operation. Conclusion: Due to systemic involvement of atherosclerosis, lower extremity peripheral artery disease should be considered in patients who underwent carotid endarterectomy operation, and patients should be evaluated accordingly.
