    Micro-dose Paranasal Computed Tomography is Valuable When Evaluating Diseases of the Paranasal Sinuses

    Aim: We explored whether a reduction in the radiation dose affected the quality of diagnostic images generated using third-generation dual-source computed tomography (CT) running advanced third-generation iterative reconstruction software. Methods: From February to May 2015, 72 consecutive patients aged 16 to 64 years with clinically suspected sinusitis were randomly and prospectively enrolled and divided into two groups (standard or micro-dose CT). Anatomical landmarks and mucosal structures were independently evaluated by a radiologist and an otorhinolaryngologist using a four-point grading scale. Mean scores were calculated and compared. Results: All images were of diagnostic quality. The mean effective radiation dose was significantly lower in patients evaluated via the micro-dose protocol (group 2) than in those evaluated using the standard protocol (group 1) (p<0.05). The mean anatomical scores showed that all landmarks were very well-defined, with the exceptions of the uncinate attachment point and the ethmoid foramen (used to identify the ethmoid artery). The Wilcoxon signed rank test revealed no significant difference between the mean scores of the two observers for either group (p>0.05). Conclusion: The radiation dose to the paranasal sinus delivered during CT in patients with suspected sinusitis can be reduced to 0.003 mSv using third-generation dual-source CT running advanced iterative reconstruction software.

    Post Endolaryngeal Laser Surgery Carbon Granuloma

    Aim: This study was performed to highlight the phenomenon of carbon granulation formation after laser vocal cord surgery. Emphasis was put on the fact that these granulomas closely resemble tumor recurrence and due to the similarity of appearance, histological evaluation is necessary for diagnosis. Methods: This study included 35 male patients presenting to the Acibadem University Hospital with early-stage laryngeal cancer between 2009 and 2013. All patients underwent laser-assisted interventions on the vocal cords. Patient follow-up was continued for two years and granuloma formation or tumor recurrence was noted. Results: During the two-year follow-up period, a total of five patients presented with new vocal cord lesions. One of the cases was determined to be a tumor recurrence. The remaining four were carbon granulomas that regressed during the follow-up period. Conclusion: Although carbon granuloma formation is not very common, it is still frequent enough to be thoroughly discussed in the literature. With laser surgery for early-stage laryngeal cancer becoming more popular every day, the incidence of carbon granulomas is likely to increase. An algorithm for this type of lesion without putting patients' health at risk will be a valuable tool in the near future.

    Reconstruction of Composit Oromandibular Defects

    Objective: Mandibular defects are generally observed as composite defects which soft tissue defects are accompanied to bone defects. This situation may cause difficulties in choosing the surgical method during the reconstruction period. The purpose of this paper is to convey our clinical approach and results on composite mandibular defects. Methods: Eleven patients, applying to our clinic between 2011 and 2015, were operated in a single session with the team of otorhinolaryngology, because of oromandibular cancer. Ten of the patients were male and one was woman. The age range was between 7 and 54 ages (Average age 35.5). Results: While mandibular resection was performed on all patients, skin resection was performed on six patients, mucosa resection was applied on eight patients

    Expression of vascular endothelial growth factor in follicular cell-derived lesions of the thyroid: Is NIFTP benign or precancerous?

    Objective: Vascular endothelial growth factor (VEGF) is an angiogenic factor that plays an important role in physiological and pathological angiogenesis of the thyroid. The aim of the current study was to determine the expression characteristics of VEGF in follicular cell-derived lesions of the thyroid and to assess whether a new entity noninvasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP) is precancerous. Material and Methods: Patients diagnosed with 33 follicular adenomas (FA), 41 invasive follicular variant papillary thyroid cancer (IN-FVPTC), and 40 NIFTP in surgical resection materials were evaluated retrospectively. Immunostaining was performed on 5-mu m paraffin tissue sections. The percentages of immunostaing for VEGF were evaluated on pathological materials.
