    360 Derece Performans Değerlendirme Sistemi ve Özel Bir Sağlık Kuruluşu Uygulama Örneği

    ÖZET İşletmelerin insan kaynakları yönetimi fonksiyonlarından biri olan performans değerlendirmede etkinliğin ve verimliliğin ölçülebilmesi son derece önemlidir. Küreselleşmenin etkisi ile değişen ve gelişen koşullara uyum sağlamak durumunda kalan işletmeler için performans değerlendirme çalışmalarının önemi artmış, hem kamu hem de özel sektörde çeşitli yöntemlerle uygulanmaya başlamıştır. Özellikle son on yılda performans değerlendirme süreçlerinin ve yöntemlerinin hızlı bir ilerleme kaydettiği görülmektedir. Örgütler ise yaşanan bu değişimlere ayak uydurabilmek için, işgörenlerin; kendileri ve üstlerinin yanı sıra diğer çalışma arkadaşları ve bazı durumlarda hizmet veya ürün sundukları müşterileri tarafından da değerlendirilmesini sağlayan 360 derece performans değerlendirme sistemini tercih etmeye başlamışlardır. Bu çalışmada amaçlanan, 360 derece performans değerlendirme yönteminin sistem içerisindeki etkisine yer vermekle beraber, diğer hastane çalışanları için uygulanabilirliği konusunda zemin oluşturmaktır. Ayrıca, Türkiye genelinde hizmet sunan özel bir sağlık kuruluşunun 360 derece performans değerlendirme sistemi detaylı şekilde incelenmiş, değerlendirme detayları ve sonuçları paylaşılmıştır.

    3D and 4D Printing of Polymers for Tissue Engineering Applications

    Three-dimensional (3D) and Four-dimensional (4D) printing emerged as the next generation of fabrication techniques, spanning across various research areas, such as engineering, chemistry, biology, computer science, and materials science. Three-dimensional printing enables the fabrication of complex forms with high precision, through a layer-by-layer addition of different materials. Use of intelligent materials which change shape or color, produce an electrical current, become bioactive, or perform an intended function in response to an external stimulus, paves the way for the production of dynamic 3D structures, which is now called 4D printing. 3D and 4D printing techniques have great potential in the production of scaffolds to be applied in tissue engineering, especially in constructing patient specific scaffolds. Furthermore, physical and chemical guidance cues can be printed with these methods to improve the extent and rate of targeted tissue regeneration. This review presents a comprehensive survey of 3D and 4D printing methods, and the advantage of their use in tissue regeneration over other scaffold production approaches.

    3D Automated Breast Ultrasound System: Comparison of Interpretation Time of Senior Versus Junior Radiologist

    Objective: This study aimed to compare the automated breast ultrasound system (ABUS) reading time of breast radiologist to a radiology resident independent of the clinical outcomes. Materials and Methods: One hundred women who underwent screening ABUS between July and August 2017 were reviewed retrospectively. Each study was examined sequentially by a breast radiologist who has more than 20 years of experience in breast radiology and third year resident who has 6 months of experience in breast radiology. Data were analyzed with Spearman' correlation, Wilcoxon Signed Ranks Test and Kruskal-Wallis Test and was recorded. Results: The mean age of patients was 42.02 +/- 11.423 years (age range16-66). The average time for senior radiologist was 223.36 +/- 84.334 seconds (min 118 max 500 seconds). The average time for junior radiologist was 269.48 +/- 82.895 seconds (min 150 max 628 seconds). There was a significant difference between the mean time of two radiologists (p=0.00001). There was a significant difference regarding the decrease in the reading time throughout study with the increase of number of cases read by the breast radiologist (p<0.05)

    A series of 3-hydroxy-6-methyl-2-((4-substitutedpiperazin-1-yl)methyl)-4H-pyran-4-on e structured compounds were synthesized by reacting 5-hydroxy-2-methyl-4H-pyran-4-one with suitable piperazine derivatives using Mannich reaction conditions. Antibacterial activities of the compounds for E. coli, S. paratyphi, S. flexneri, E. gergoviae, and M. smegmatis were assessed in vitro by using broth dilution for determination of the minimum inhibitory concentration (MIC).     5-Fluorouracyl added infusion fluid in patients with recurrent rhegmatogeneous retinal detachment

    Purpose: To compare the efficacy of 5-fluorouracil (5-FU) added the infusion fluid with a control group in the event of grade C PVR in recurrent retinal detachment (RD). Methods: The records of the patients with recurrent retinal detachment with grade C PVR who underwent vitrectomy for retinal detachment surgery between April 2003 and October 2004 were reviewed retrospectively for this comparative study. The recurrent retinal detachment patients with grade C PVR who underwent vitrectomy and had a minimum post-operative follow-up period of 12 months were included. The patients were divided into two groups as study and control groups. 5-FU (200 microgram/ml) and low-molecular-weight-heparin (LMWH) (5 IU/ml) was added into the infusion solution of the study group. Primary outcome measure of this study was the single operation anatomical success at month 12. Results: A total of 43 eyes of 43 patients were included. The control group was consisted of 26 eyes (60.5\%) and the 5-FU group was consisted of 17 eyes (39.5\%). At month 12, single operation anatomical success was obtained in 14 of the 26 patients (53.8\%) in the control group and in 16 of the 17 patients (94.1\%) in the 5-FU group (p = 0.005). Conclusion: Favorable outcomes were obtained in the patients with recurrent RD and grade C PVR in whom 5-FU and LMWH added infusion fluid which was used during vitrectomy.

    5-Hydroxyuracil Incision Activity Varies According to the Histological Grade of Non-muscle-invasive Bladder Cancer

    Objective: High levels of endonuclease III-like 1 (NTHL1) DNA glycosylase, which plays a role in the first step of the base excision repair pathway, has been related to cancer initiation and progression. 5-hydroxyuracil (5-OHU) oxidative base damage is a substrate for NTHL1 and endonuclease VIII-like 1 enzyme 1 (NEIL1) DNA glycosylases. This study investigates the association of 5-OHU incision activity with the risk of disease progression in patients with non-muscle-invasive bladder cancer (NMIBC) regarding grade and stage. Materials and Methods: During transurethral resection of 17 NMIBC patients, the papillary tumour before monopolar resection and healthy bladder mucosal tissue from the same person were obtained using cold cup biopsy. Both the normal mucosa and NMIBC tumour were pathologically confirmed. The histological grade and stage were also determined. The 5-OHU incision activity of all tissues was measured using a radiolabelled 5-OHU modified base containing DNA substrate. Results: 5-OHU incision activity was significantly higher in all high-grade NMIBC tissue extracts compared with the corresponding normal tissues (p=0.001). However, we found no significant difference in 5-OHU incision activity in low-grade NMIBC tissues (p=0.89). There was also a significant increase in 5-OHU incision activity at the Ta/T1 stage compared with the corresponding normal tissue (p=0.001). Conclusion: The increase in 5-OHU incision activity according to the histological grade of NMIBC tissue indicates that this activity (mainly performed by NTHL1 and NEIL1 DNA glycosylases) might play a role in NMIBC prognosis. Thus, it could be used as a potential prognostic biomarker for NMIBC.

    73 yaşında Kadın Hastada Ateroklerotik Kalp Hastalığına Eşlik Eden Cor Triatriatum Sinister

    ÖZET Cor triatriatum sinister oldukça nadir görülen bir konjenital kalp hastalığı olup sol atriumun, fibromüsküler membran ile iki faklı bölmeye ayrılması ile karakterizedir. İlk olarak 1868’de Church tarafından rapor edilmiştir (1). Oldukça nadir olarak gözlenen bu kardiyak anomalinin insidansı pediatrik yaş grubunda %0.1-0.4’tür. Erişkinlerde çok daha nadir olarak görülmekte olan cor triatriatumun tanısı ise sıklıkla rastantısal olarak konulmaktadır (2). Klasik olarak, hastalara neonatal dönemde veya erken infant dönemin de tanı konulsa da bazı vakalar erişkin döneme kadar saptanamayabilmek tedir. Vaka sunumumuzda, koroner arter hastalığı nedeniyle cerrahi plan lanan ve preoperatif incelemede rastlantısal olarak cor triatritum sinister saptanan 73 yaşındaki kadın hastadan bahsedilmiştir.

    786T/c endothelial nitric oxide synthase gene polymorphism and coronary collateral circulation

    Introduction: In this study, we investigated the association between -786T/C polymorphism of the endothelial nitric oxide (NOS3) gene in which thymidine is replaced by a cytosine at nucleotide -786 (rs 2070744) and coronary collateral circulation (CCC) in patients with stable coronary artery disease. Materials \& Methods: 286 patients having a critical stenosis (> 95\%) in at least one major epicardial coronary vessel were included in the study. CCC was defined according to the Rentrop classification (R). Patients with R0-1 CCC were included in the poor CCC group and subjects with R2-3 CCC were assigned to the good CCC group. The polymerase chain reaction method was used for genotyping. 152 patients with poor CCC and 134 patients with good CCC were examined. Results: The frequency of cytosine-cytosine (CC) and thymidine-cytosine (TC) genotypes and allele C were higher in the poor CCC group, but the difference did not reach statistical significance. In the dominant model, the frequency of CC+TC vs. thymidine-thymidine (TT) genotypes was significantly higher in the poor CCC group (67.1\% vs. 54.5\%, respectively

    A Biomechanical Comparison of Tendon Repair with a Knotless Barbed Suture and a Conventional Monofilament Suture Material: An ex-vivo Animal Experiment

    Objective: Our aim is to compare the ultimate tensile strength and stiffness of a 2/0 barbed suture and a 3/0 polypropylene monofilament suture in a porcine tendon repair model. Methods: Sixteen porcine Achilles tendons were transected and separated into two groups. In group I tendons were repaired with a modified knotless four-strand Kessler technique using a 2/0 V-Loc barbed suture. In group II tendons were repaired with a four-strand Kessler technique using a 3/0 monofilament conventional suture and knots were tied. All specimens were biomechanically tested for ultimate tensile strength (UTS) and stiffness. Mode of failure was also noted. Results: Five specimens in group I failed by stripping of the suture from the tendon tissue, three failed due to suture breakage. In group II two out of eight tendons failed by stripping of the suture and remaining six failed by suture breakage. Median UTS value was found to be 85.96 N (range: 63.24) in group I and 64.29 N (range: 56.84) in group II. Median stiffness value of the samples in group I was found to be 5.67 N/mm (range: 4.32) and in group II it was found to be 4.53 N/mm (range: 6.23). The statistical analysis of UTS and stiffness values revealed no significant difference between the groups (p=0.17 and p=0.56 respectively). Conclusion: A knotless Kessler tendon repair made with a 2/0 barbed suture is biomechanically equivalent to a knotted Kessler tendon repair made with a 3/0 conventional polypropylene suture in ex-vivo conditions.

    A biosensor platform based on amine functionalized conjugated benzenediamine-benzodithiophene polymer for testosterone analysis

    A novel benzenediamine-benzodithiophene polymer is synthesized for use in biosensor fabrication for the detection of testosterone. The sensory platform is constructed via drop coating on a screen-printed carbon electrode, using poly(benzenediamine-Bis{[}(2-ethylhexyl)oxy]benzodithiophene) (pBDBT) as the polymer layer. Testosterone antibodies are immobilized on the polymer-coated electrode surface via glutaraldehyde, which binds to the surface through the amino functional groups on the polymer backbone. The changes in the surface features due to testosterone binding are investigated via electrochemical techniques such as differential pulse voltammetry, cyclic voltammetry, and electrochemical impedance spectrometry as well as contact angle measurements. Surface morphology of the modified electrodes is characterized by atomic force microscopy. The linear range and limit of detection of the sensor are calculated. Impact of possible interfering compounds is investigated. Furthermore, the sensory platform is utilized for testosterone analysis in synthetic biological fluids.
